Masseter Botox Aftercare + Recovery Guide

Higher-dose treatment in a strong muscle. Standard Botox aftercare plus a few masseter-specific notes.

First 24 Hours

Do:

  • Stay upright for 4 hours after treatment
  • Apply ice if you notice any swelling at the injection sites
  • Eat softer foods if your jaw feels weak in the first day or two
  • Take Tylenol if needed for discomfort

Don't:

  • Touch, rub, or massage the treated area
  • Lie down or bend over for 4 hours
  • Exercise vigorously for 24 hours
  • Drink alcohol for 24 hours
  • Take blood-thinning medications (aspirin, ibuprofen) unless prescribed
  • Bite into very hard foods (crusty bread, raw carrots, ice) for the first few days

Week 1: What's Normal

  • Mild jaw soreness or tenderness when chewing — typically resolves in 3-7 days
  • Subtle weakness in your bite force as the muscle starts to relax — this is the treatment working
  • TMJ relief often noticed within the first week if jaw tension was your concern
  • No visible slimming yet — that builds gradually over 4-6 weeks as the muscle atrophies from disuse

The 4-6 Week Slimming Build

  • Weeks 2-3: Subtle jaw contour change as muscle bulk reduces
  • Weeks 4-6: Visible slimming. Your jawline appears softer, more defined.
  • Months 3-4: Muscle activity gradually returns. Most clients book session 2 around month 4 to maintain or compound the slimming.
  • Long-term: 2-3 sessions over 6-12 months produces sustained slimming. After that, maintenance every 6 months is typical.

If You Grind or Clench

Many masseter Botox clients are treating bruxism (clenching/grinding) as much as slimming. Continue using your night guard if you have one — Botox reduces but doesn't eliminate the muscle's force. Some clients report the relief is dramatic; others notice subtle improvement.

How Long Does Masseter Botox Last?

Masseter Botox typically lasts 4-6 months — longer than cosmetic Botox because the masseter is a large, strong muscle and uses higher unit doses. Patients who clench or grind heavily may need more frequent treatments (every 3-4 months).

  • Muscle bulk: larger masseter muscles hold the slimming effect longer
  • Clenching / grinding pattern: heavy bruxers may metabolize Botox faster due to constant muscle activity
  • Dose per side: 20-30 units per side typical; under-dosing limits both slimming and TMJ relief
  • Treatment cadence: every 4-6 months early on; spacing can extend after 2-3 sessions as the muscle atrophies

When to Contact Us — Specific Clinical Criteria

Call us right away if you experience any of the following. These are specific thresholds — vague concerns can wait until business hours, but anything on this list is urgent:

  • Difficulty swallowing or chewing solid food beyond day 7
  • Difficulty breathing — call EMS, then us
  • Asymmetric smile lasting > 14 days
  • Significant cheek hollowing or unwanted volume loss in the upper face
  • Fever ≥ 101°F + expanding redness > 2 cm at injection site
